Program Summary:
Bay House is a 12 bed short-term residential facility designed to assist individuals with serious and persistent mental health issues to learn to live more independently in the community. Each resident has a bedroom shared with one peer with common living spaces. Experienced and qualified staff are on site 24 hours a day.

Program Description:
Bay House is a 12 bed short-term residential facility designed to assist individuals with serious and persistent mental health issues to learn to live more independently in the community. Each resident has a bedroom shared with one peer with common living spaces. Experienced and qualified staff are on site 24 hours a day.

Bay House is a transitional program which emphasizes empowerment and individualized treatment. Services provided to individuals while residing at Bay House may include life skills management (shopping, budgeting, cleaning, personal hygiene and learning the transportation system), medication management, supportive counseling, social outings/leisure activities training, educational groups, dual diagnosis groups, crisis intervention and community reintegration. All individuals are required to actively participate in activities during the day, such as attend Turning Point Psychosocial Rehabilitation, Charlotte's Place Clubhouse or Supportive Employment programs. Most residents also receive the services of a Case Manager.

After staying at Bay House for up to 90 days, 80 percent of clients are able to move into their own apartments or other less-restrictive environments, reducing hospital stays.
